3.6     Information Requirements.  Each request shall include the following
information:
   
(a)     Customer s name, address, representative and telephone number for both
statements and invoices.
   
(b)     A statement of whether Customer is a local distribution company (LDC),
an intrastate pipeline company, an interstate pipeline company, a
marketer/broker, producer, end user or other type of entity (which other
entity must be described).
   
(c)     A statement of whether Customer is affiliated with Caledonia.
